A levy sending procedure

When sending specimens, they must be identified by the animal's complete identification number and accompanied by a record (accompanying document).


The record must include the following information:

  • The contact details of the owner or host of the animal,
  • The contact details of the person requesting the test,
  • The contact details of the payer,
  • Complete identification of the animals from whom specimens were collected (National identification number, Name of animal, date of birth, etc).
  • The date of specimen collection (mandatory  information)
  • Specify the test(s) you require. To do this, please consult our catalogue.
  • Check the type of specimen relevant to the test here.
  • Requester's signature
